LIABILITY WAIVER & RELEASE AGREEMENT

By checking the waiver box during registration, I acknowledge that I have read, understood, and agreed to the terms of this Liability Waiver & Release Agreement.

I voluntarily choose to participate in dance classes, workshops, socials, events, and related activities offered by Latin Dance Toledo LLC. I understand that participation in dance and physical activities involves inherent risks, including but not limited to slips, falls, physical exertion, collisions, sprains, strains, and other injuries that may occur during participation.

I knowingly and voluntarily assume all risks associated with participation in these activities.

I acknowledge that I am responsible for consulting a physician before participating in any physical activity and confirm that I do not have any medical or physical condition that would prevent safe participation.

To the fullest extent permitted by law, I release and hold harmless Latin Dance Toledo LLC, its owners, instructors, employees, volunteers, agents, affiliates, landlords, and event venues from any claims, liabilities, damages, injuries, losses, or expenses arising out of or related to my participation in studio activities, except in cases of gross negligence or intentional misconduct.

I understand that classes and events may be photographed or recorded for promotional and marketing purposes. I grant permission for my image, voice, or likeness to appear in photos or videos used by Latin Dance Toledo LLC without compensation or additional approval.

In the event of an emergency, I authorize reasonable emergency medical care if necessary and understand that I am solely responsible for any related medical costs.

If the participant is under 18 years of age, I certify that I am the parent or legal guardian and agree to these terms on behalf of the minor participant.

If any portion of this agreement is determined to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
